Boarding Agreement
This is a Contract between Xcaliber Stay and Play and Pet Owner whose signature appears below
1. By signing this agreement and leaving your pet with our kennel, Owner certifies to the accuracy of all information given about their pet. Including but not limited to vaccination due dates and sociability of pet.
Xcaliber Stay and Play. reserves the right to deny admittance of any pet at any time for any reason.
2. Owner agrees to pay for all boarding fees at current rate at check in. Also including charges for any special services provided, and all Veterinary costs for the pet during the time it is in the care of the kennel. Pet shall not leave the kennel until all charges are paid in full.
3. Owner specifically represent that they are sole owner/ owners of the pet free and clear.
4. Before pet is accepted for boarding, training or day stay (day care), Owner specifically represent to kennel that pet has not been exposed to any contagious diseases within 30 days period prior to check in . Owner must provide Veterinary certification of listed current vaccinations.
- DOGS : DHLPP vaccine (Distemper, Parvo, Hepatitis, Parainfluenza), Rabies vaccine, and Bordatella (kennel cough). Due to the fact that Bordatella is not 100% effective. It is recommended that nasal administration be given at least 3 days prior to boarding. Injection administration be given 10 days prior for maximum effectiveness. Xcaliber Stay and Play. makes no guarantees regarding your Veterinarian's Bordatella vaccine's effectiveness.
- CATS : Rabies vaccine, Distemper vaccine (Feline Viral Rhinotracheitis, Calicivirus, Panleukopenia, Chlamydia), and Feline Leukemia vaccine or certification of a negative test.
5. Pocket pets (bunny, Bird, hamsters, Ect) brought for boarding MUST BE PRE-PAID at check in.
6. For your pets health and others we strongly recommend your pet be treated with Flea/Tick extermination treatment.
7. There will be a $10.00 charge for destroyed bedding that was rented from the kennel.
8. Owner of two pets boarding understand that, even if their pets get along fine at home, on occasion we have experienced some family pets do not enjoy boarding together. They may be aggressive toward one another or our staff. For the safety of our staff and your pets, Xcaliber Stay and Play. has the right to separate your pets.
9. Xcaliber Stay and Play
can not be held responsible for loss or damage to personal belongings that are brought into the kennel.
10. If a pet becomes ill and requires professional attention, the kennel, in its sole discretion, may engage the services of a veterinarian, administer medicine or give other requisite attention to your pet. The expenses thereof shall be paid by owner.
11. All charges incurred during the pets stay are payable upon check-out. If owner does not check-out the pet or make acceptable arrangement with Xcaliber Stay and Play within 5 days after the day the pet was due to be checked out. The pet shall be deemed abandoned and become the property of Xcaliber Stay and Play. and will be turned over to the PA Dog Law Enforcement Officer or any other party seen fit. Abandonment criminal charges will be filed against the owner (s) and all fees due.
